MEDICATION OPTIMIZATION GeriMedRisk is a network of specialists that support clinicians involved in the care of older adults. When clinicians have a question about their patients' medications, they can access GeriMedRisk's team of specialized pharmacists and physicians by telephone or e-consult. The service optimizes patients' medication plans and decreases drug-related cognitive impairment, falls, and hospital visits. The service is particularly crucial for those in rural or remote areas. CABHI'S ROLE: CABHI funding allowed the GeriMedRisk team to test and evaluate their service over 12 months in four long-term care facilities across the Waterloo- Wellington region. CABHI also supported the team in preparing a successful proposal for the Ministry of Health and Long-Term Care, resulting in $1.5 million in base funding. SMART BED SHEET The people most at risk of bedsores (also called pressure ulcers) are those with limited ability to change positions or those who spend most of their time in a bed or chair – namely, older adults. Startup Curiato is tackling the issue with their innovative skin-data platform. The system uses a bedsheet with built-in sensors to monitor patients and deliver data to care teams in real time, so pressure injuries can be accurately predicted and prevented. CABHI'S ROLE: CABHI matched Curiato with two Ontario acute and long-term care institutions to test their solution with individuals living with dementia. CABHI is also currently providing Curiato advisory services to advance their business model and prepare them for market launch.
