COMPLEX CONTINUING CARE The Complex Continuing Care (CCC) Program is offered in a non- acute hospital setting for older individuals who have complex medical conditions. It provides assessment, treatment and care for patients with multiple chronic complex medical conditions and diagnoses. We use a collaborative approach with patients and family to set goals and manage the needs of patients in a way that promotes an enhanced quality of life. An application can be sent by the patient's healthcare team in acute care through the Resource Matching and Referral (RMR) system or via fax to 416-785-2471. PALLIATIVE CARE The Shirley and Philip Granovsky Palliative Care Unit is an in-patient program providing end of life care to patients with late or end-stage disease. The interprofessional team provides pain and symptom management and emotional and spiritual support to patients. The Palliative Care Unit may also provide patient admissions for short term pain and symptom management.
