AMBULATORY MENTAL HEALTH CLINIC
Baycrest's interprofessional health care team provides assessment, consultation,
and treatment to older adults with complex medical and psychosocial issues.
Individuals over the age of 65 who attend appointments at Baycrest can receive
these services, as well as follow-up care, case coordination and education and
support.

GERIATRIC PSYCHIATRY COMMUNITY SERVICE (GPCS)
The GPCS is a community outreach program that provides assessment, treatment
and care to homebound individuals in the community. Services include education
and caregiver support and counselling.

PSYCHIATRIC DAY HOSPITAL
The Psychiatric Day Hospital offers a multi-component, holistic approach to
individuals aged 65 and over experiencing psychiatric illness such as mood
and anxiety disorders. Treatment includes individual and group psychotherapy,
medication management, and a variety of other group interventions on an
outpatient basis. Participants attend the program for either 2 days/week or 4
days/week for 16 weeks.
A referral from a physician or nurse practitioner is required. Referral forms are
available in the Clinical Services section at baycrest.org. Completed forms must be
faxed to Outpatient Services Centralized Intake at 647-788-2199.
