BAYCREST VIEWBOOK 2021 | 79 ONTARIO HEALTH TEAMS Baycrest is proud to be a core partner in two Ontario Health Teams announced by the Ministry of Health in December 2019 – the North Toronto Ontario Health Team (OHT) and the North York Toronto Health Partners. We are in a unique position to participate in two Ontario Health Teams given that we serve as a regional resource. OHTs are a new model of care that bring healthcare providers together to work as one team. The North Toronto OHT is designing an integrated care delivery model focused on ensuring seamless navigation and coordinated care for frail seniors with low, rising and high-risk needs. With our North York Toronto Health Partners, we are designing an integrated care delivery model intended to reduce inappropriate care, unwanted admissions, and ultimately, hallway medicine, with an initial focus on frail seniors, individuals with mental health and addiction issues, and those at end of life. INTERPROFESSIONAL PRIMARY CARE TEAM (IPCT) Baycrest is the lead partner in the North Toronto Interprofessional Primary Care Team, which works with family physicians in solo-practice to care for more complex patients in the community. The team increases patient attachment to primary care and offers equitable access to interprofessional care. Partners include: Baycrest, Ontario Health Central Region Home and Community Care, North Toronto Ontario Health Team, Toronto North Support Services, Alzheimer Society of Toronto, Sunnybrook, the Otter Creek Family Health Organization and many other family physicians in the area. The team provides interprofessional assessment and treatment in offices and community settings, virtually, and at home. The team includes: Nurse Practitioner, Occupational Therapist, Physiotherapist, Social Worker, Mental Health Case Manager, Pharmacist, Care Coordinator, First Link Care Navigator, and Dietitian and can facilitate access to Chiropody, Dentistry, Speech Language Pathology and Audiology.
