6 • BrainMatters • Spring/Summer 2022 Baycrest's quest to redefine the way individuals live with dementia has taken a major leap forward with the opening of the new Baycrest Terraces Memory Residence in April 2022. The new memory care residence at Baycrest Terraces uses the highly innovative, evidence-based Possibilities by Baycrest™ approach to care and program delivery, which recognizes a person's abilities and their possibilities, rather than focusing on their inabilities. It is the first time the new care model is being used in a residential setting. "This is an exciting and significant step in our journey to transform the way we support individuals living with dementia, and their caregivers, across all of Baycrest's clinical, community and congregate residential settings," says Baycrest President and CEO Dr. William Reichman. Choice, Purpose and Autonomy Drawing on 104 years of expertise, our transformational care approach, Possibilities by Baycrest™, shifts the residential experience away from a traditional focus on daily tasks to one that maximizes the abilities of each individual living with dementia. Staff and the care community are enabled to offer individuals living with dementia a personalized, flexible, meaningful experience that recognizes the person first through the nurturing of a relationship, while applying solutions that leverage our knowledge of the brain. Despite any reduced capacity to make healthcare or financial decisions, an individual with cognitive changes will continue to be able to make choices about how they spend their day, including what to eat; what activities to participate in; who they spend time with; and, what time to go to bed. Evidence-based Solutions "Our model introduces a new concept, the Possibilities Plan, which goes alongside a traditional care plan," explains Faith Malach, Executive Director, Care Innovations and Transformation. "It leverages Baycrest's expertise and knowledge of the brain to inform and enable practical solutions, use of technology, and group and individual programming."
