BAYCREST HEALTH SCIENCES www.baycrest.org 3 M YC H A R T P I LOT L AU N C H E S New patients at the Sam and Ida Ross Memory Clinic can now access their medical records anywhere using a tool called MyChart. The online application makes it easy for patients to view their personal health information and enables secure data sharing among members of the care team. Patients at the clinic who are using the new system are part of Baycrest's MyChart pilot project currently underway in partnership with Sunnybrook Health Sciences Centre. T H E B AYC R E S T M O D E L A key part of our Strategic Plan, 'The Baycrest Model' has taken shape over the past year in consultation with staff, clients, family members and external subject matter experts. It emphasizes Baycrest's unique strengths and focuses on empowering clients and families, holistic practices, knowledge transfer, technological innovation and an ongoing commitment to our culture. Over the coming year, we'll begin a pilot implementation of the Baycrest Model, using it as the foundation for the unique, integrated and person-centred care experience we provide to our clients, in the community, in long-term care or in the hospital.
