April 1, 2014- March 31, 2015 ● KLAERU-PE Annual Report Page 11 FOLLOW-UP OF THE BUILDING CAPACITY IN UNDERSTANDING BURDEN OF ILLNESS IN CHRONIC CARE AND LONG-TERM CARE Dr. Daphna Grossman, Dr. Giulia Perri, Dr. Jurgis Karuza, Anna Berall (2014- 2015) This study proposal addresses a quality improvement initiative to implement a palliative screening tool on diverse geriatric inpatient units and in the long-term care facility at Baycrest. The purpose of this study is to better understand the wider scope of application for a palliative screening tool. The Gold Standards Framework Prognostic Indicator Guideline© (GSF-PIG©) tool will serve as the intervention to screen patients to understand a patient/resident's disease trajectory thus alerting the need for Goals of Care (GoC) and Advanced Care Planning (ACP) discussions. The PEU oversaw the development and implementation of the study. The PEU wrote the study proposal, submitted the REB documents, created the online surveys in collaboration with the Principal Investigator, placed 5 volunteers with the project, reviewed medical charts, administered interviews, conducted data analyses, and assisted with writing and formatting a presentation. This project has had local and international presentations: American Medical Directors Association, Louisville Kentucky, 2015; Canadian Hospice Palliative Care Conference, Ottawa, ON, 2015; The Canadian Society of Palliative Care Physicians, Calgary, AB, 2015. A RETROSPECTIVE CHART REVIEW OF 100 PATIENTS DISCHARGED FROM THE LONG TERM PSYCHIATRIC INPATIENT UNIT Dr. Matt Robillard, Dr. Paul Katz, Anna Berall (2014) This report was in response to a request from Medical Services to provide additional detail for a report on the inpatient psychiatry unit (4 East). This was a retrospective chart review of 100 registered patients admitted to the psychiatry unit from February 2013 to July 2014. A summary of patient characteristics and outcome data, and correlations with clinical team assessments were provided. The chart review also included outcome data from the Ontario Mental Health Reporting System (OMHRS) Annual Admission Assessments. Under the leadership of Anna Berall, the PEU developed the database, oversaw a volunteer in the chart review and data collection, conducted statistical analyses, and prepared a presentation for clinical staff. A PowerPoint presentation detailing the study was provided to the clinical team in August 2015. Interactions among Baycrest staff, patients and families.
