8 Since December 2010, the PEU has supported evaluaon projects spanning years of work with resounding success. The following are some highlights: PEU Project Highlights The PEU responds to program evaluation requests from Baycrest leadership, which is vetted by the Office of the Vice President for Medical Services SLOW STREAM REHABILITATION: PREDICTOR OF OUTCOMES 4 MONTHS POST DISCHARGE Dr. Paul Katz, Dr. Gary Naglie, Dr. Jurgis Karuza, Angela Chan, Grace Leung, Anna Berall (July 2011‐present) This study was an AFP‐funded project that addressed the need for knowledge in caring for the more frail populaon characterizing slow stream rehabilitaon programs. It will also define the predicve validity of assessment tools by following a cohort of slow stream rehab paents and determining their status (death, rehospitalizaon, admission to a nursing home, community living) four months post discharge. The PEU wrote the study proposal, completed REB applicaons, and engaged and assisted the clinical team in the development of the study methodology. The PEU oversaw study volunteers, recruited 105 paents, conducted paent interviews, trained clinical staff, and conducted inter‐rater reliability tesng for funconal measures, and completed data analysis, study abstracts, conference posters, and a manuscript published in the Journal of the American Medical Directors Associaon. INVESTIGATING END STAGE LIVER DISEASE IN A GERIATRIC PALLIATIVE CARE UNIT Dr. Giulia Perri, Dr. Daphna Grossman (June 2013‐2016) End Stage Liver Disease (ESLD) involves progressive decline and de‐ compensaon of liver funcon. This study aims to characterize the demographics and illness trajectory of paents with ESLD in order to beer serve this growing populaon. The PEU spearheaded the design of a retrospecve chart review which was conducted for paents admied to the palliave care unit at Baycrest. Addionally, the PEU facilitated data analysis and provided support for numerous conference abstracts and presentaons including the 2016 Annual Hospice Palliave Care Ontario Conference in Toronto, ON, and the 12th Annual Advanced Learning in Palliave Medicine Conference in Calgary, AB.
