26 Saul and Rae Saltzman Day Treatment Centre (DTC): The Day Treatment Centre provides a rehabilitation program for older adults living in the community with complex medical conditions. Our team is interdisciplinary in nature and goal oriented. Our primary aim is to assist our clients in remaining independent as long as possible. The cost of the program is covered by OHIP. Program details Three-hour sessions, twice a week for up to 12 weeks. Clients who meet their goals sooner do not have to attend for the full 12 weeks. Services provided: • Medical and Nursing Assessments • Health Education for Patients, Families and Caregivers • Social Work • Physiotherapy • Occupational Therapy • Discharge Planning • Group and Individual Exercises • Referral to Baycrest Services (i.e. Urology, Audiology, Speech Language Therapy, Swallowing, Leisure Education) The following specialist services are available to patients referred by a physician. Ambulatory Mental Health Services Ambulatory Mental Health Services encompass on-campus care and outreach teams; both provide interprofessional psychogeriatric assessment. From case management to day hospital programs, patients receive tailored services such as the use of problem solving therapy and other forms of individual and group psychotherapy, as well as prevention and wellness strategies in order to meet their mental health goals.
