pathology, pharmacy and nutrition. Other professionals are available on a consultative
basis.
TRANSITIONAL CARE UNIT
The Transitional Care Unit (TCU) at Baycrest Hospital has 21 beds. It serves as a
bridge for Baycrest inpatients who are preparing for discharge from the hospital to an
alternate level of care.
The goal is to provide an easier transition between the hospital and a long-term care
facility. Our staff works with patients and families to ensure that their needs are met
and that health status remains stable.
The interdisciplinary team includes physicians, nurses, social work, pastoral care,
dietitians and a therapeutic recreationist. Other professionals are available for
consultations.
