CREATED Evaluation Tools KL-CARE introduced and piloted the "Preparedness for Caregiving Scale" to evaluate ICCT caregiver support; this measure has since been adopted in ICCT clinical practice. INCREASED Team Knowledge KL-CARE's efforts enabled the team to better understand the activities that were completed during a home visit, while identifying that an underserved population of frail, older adults is now able to access geriatric care. PROMOTED Evidence-based Care The results of this evaluation strengthened and clinically validated the impact of this innovative care approach on ICCT members, patients, caregivers, and community-based physicians. KL-CARE'S CURRENT INVOLVEMENT WITH THE ICCT Due to the success of this ongoing initiative, KL-CARE is currently evaluating an ICCT Telemedicine project where teleconferencing is being used to provide clinical healthcare from a distance. AFFECTED Clinical Practice KL-CARE collaborated with the ICCT to develop and implement the "Unpacking a Home Visit" form, which categorizes the services carried out during a home visit. Moser, MD, MSc, CCFP (COE), FCFP, CMD Associate Medical Director, The Jewish Home for the Aged/Apotex, Family Medicine, Baycrest The expertise and support of the KL-CARE team for the evaluation of this clinical team has been invaluable. With their guidance, we have been able to evaluate components of the ICCT to help drive continuous quality improvement and client outcomes.
