KL-CARE Annual Report 2016-2017 Page 21 PATIENT-CENTRED FALLS PREVENTION KL-CARE'S IMPACT The KL-CARE team found that patients were able to increase their access to care and support surrounding falls prevention, increasing their understanding of their own falls risk. PROMOTED PATIENT-CENTRED CARE The KL-CARE team discovered that through this intervention, patients could work with their families and healthcare team to create individualized, falls- prevention strategies to reduce falls risk. CONFIRMED THE FEASIBILITY OF TAILORED CARE PLANS KL-CARE's analysis indicated that nurses reported increased confidence in their ability to provide care as a result of this program. HIGHLIGHTED INCREASED CLINICIAN CONFIDENCE KL-CARE also mentored a Registered Nurses' Association of Ontario (RNAO) fellow: KL-CARE developed face-valid satisfaction surveys to assess patients' engagement in their own care. CAN BE USED TO EVALUATE SIMILAR PROGRAMS Deborah Lappen, RN, MN, GNC(C), Baycrest KL-CARE has guided me through two nursing fellowships, including writing and publishing a manuscript. They helped me to create and carry out a meaningful project that aligned with a falls-prevention program. KL-CARE supported my goals and allowed me to grow in my career.
