9 EVALUATION OF BEST PRACTICES FOR CONTINENCE ON A SLOW STREAM REHAB UNIT Debbie Lappen, Amy Davignon (2014‐2016) Baycrest Health Sciences has been undergoing evaluaon to become a Best Pracce Spotlight Organizaon© with the Registered Nurses' Associaon of Ontario (RNAO) since 2012. Baycrest selected connence as a guideline for implementaon based on the incidence of connence in our geriatric populaon. Using guidelines for connence care management with early assessments and an interdisciplinary team approach improves quality of life, as well as paent outcomes with decreased inconnent episodes, and lowers the risk of infecon, delirium, falls, and skin breakdown. The purpose of this study was to evaluate the implementaon of connence guidelines and strategies for managing connence, as integrated into clinical pracce for a geriatric rehabilitaon inpaent populaon. To date, the PEU has successfully facilitated survey design and administraon, data analysis, and abstract submissions. The PEU is currently providing manuscript support. EVALUATION OF AN INTEGRATED COMMUNITY CARE TEAM (ICCT) Dr. Sid Feldman, Dr. Gary Naglie (Baycrest); Jodeme Goldhar (Toronto Central CCAC) (2014‐ 2016) Baycrest, together with the Toronto Central Community Care Access Centre (TC CCAC) and North York General Hospital (NYGH) are evaluang the Integrated Community Care Team (ICCT) for the BRIDGES iniave. The ICCT provides a unique, comprehensive spectrum of integrated care for frail, older adults who: 1) are at high risk for hospitalizaon, 2) are cared for by a "solo" primary care physician or do not have a primary care physician, and 3) may have difficulty accessing tradional office‐based primary and/or specialist care due to cognive, physical, or funconal issues. To date, the PEU has facilitated the project's evaluaon including study design, grant applicaons, REB approval, data collecon & analysis, volunteer supervision, stakeholder management, and project reporng. BAYCREST MODEL: PILOT FOR LEISURE ENGAGEMENT FOR ACTIVE & SPONTANEOUS EXPERIENCES (PLEASE) Karen Hirshfeld, Janis Sternhill, Erin Corey The purpose of this study is to evaluate a new approach and model for therapeuc recreaon service on the fih floor of the Apotex Centre at Baycrest. This project incorporates the guiding principles of the Baycrest Model of Client Experience and the current literature in the field of therapeuc recreaon, including: person‐centred and relaonship‐centred care, partnership approach, and quality of life. The evaluaon will include administraon of pre‐post surveys and compleon of focus groups as well as training of volunteers to collect observaonal rangs of resident engagement. This is a pilot study using a modified observaon rang tool to rate residents' acvies. To date, the PEU has successfully facilitated survey design & administraon, data collecon and analysis, submission of an abstract for a poster presentaon, and preparaon of reports. The PEU partners with Baycrest Volunteer Services, providing opportunies for students to engage in research at Baycrest. COMMUNITY
